Payment of your estimated patient portion is due at the time services are rendered unless other arrangements have been made in advance.

Insurance:

If you have dental insurance, our highly trained staff will do their best to ensure that we maximize your coverage for procedures performed in our office, and that we make the claims process as easy as possible for you.

We participate with many dental insurance plans including: Aetna (DMO and PPO), Anthem Blue Cross, Assurant, Benecare, Cigna (DMO and PPO), Delta Dental, Dental Health Alliance (DHA), GE, Fortis, Genworth, Sunlife, Guardian, Principal, United Concordia, Unum, Wellpoint, and many others.  

Even if we aren't a participating provider for your specific coverage plan, our office will still accept assignment of benefits from most dental plans.

Once you provide us with your insurance policy and subscriber information, we will be happy to verify our status as an in-network provider for your specific plan.

Co-Pay Estimation:

When you schedule an appointment with our office, we will ask you for the policy and subscriber information of any dental insurance plans which might provide coverage for your treatment. Prior to your visit, every effort will be made to verify your coverage and provide you with an accurate estimate of your expected co-pay (which will be due at the time treatment is initiated).

Once your treatment has been completed, we will submit a claim to your insurance provider. If your insurance carrier pays more than we estimated, you will immediately be sent a refund of your overpayment. If they pay less than estimated, you will be responsible for any remaining balance on your account.

Why only an Estimate?

We calculate your estimated co-pay based on the information provided to us by your insurance carrier when we verify coverage and benefits. Although we do our best to ensure the accuracy of our estimates, they are not a guarantee of coverage or payment by your insurance company. Ultimately, you will be responsible for any portion of the fee not covered by your insurance.

There are several factors, beyond our control, which may impact the accuracy of our estimates. The final amount of your responsibility may differ from our co-pay estimate for many reasons including: incorrect, incomplete or inaccurate information provided to us by your carrier; “maxing out” on plans with set yearly benefit limits; or frequency limitations placed by some plans on the number of times a given procedure will be covered in a calendar year.

If you receive an Explanation of Benefits (EOB) from your insurance company or a statement from our office showing that your insurance did not cover what was expected, please don’t hesitate to contact our office. We will be happy to investigate the matter and help you understand why there may be a difference between the actual amount covered and the estimated insurance contribution.
